Do any errors show up in your browser console? Try pulling up your browser's dev tools (usually the F12 key), then go to the Console tab. After trying to upload an attachment, see if there are any error messages there. You may also need to check the Network tab as well. After the upload, there should be a server response entry, click it, go to the Response tab and see if the output shows any kind of error message (it may be JSON formatted).
